Please be sure to bring these two items:
• One form of Photo ID (e.g., driver’s license, passport)
• Your current Insurance Card
The most important initial step is completing our intake form. You can choose to:
• Fill it out online before you arrive
• Fill it out in person when you check in
You will meet two key people during your visit:
• Our Front Desk Person (for check-in and administrative items)
• Dr. Kureshi (for your consultation and care)
A $25 fee will need to be paid at the time of scheduling a med spa service appointment. The $25 fee will be credited towards any med spa procedure that is performed. However, if you cancel the appointment with less than 48 hours ntoice or don’t make your appointment the $25 fee will be non-refundable.
